Title
Dietitian’s Approach to Managing Enteral Nutrition Intolerance When a Formula Change is Indicated: A Canadian Clinical Practice Survey
Introduction/Purpose
Enteral nutrition intolerance (ENI) is often defined as one or more gastrointestinal (GI) symptoms related to enteral nutrition (EN) and may have significant impact on patient outcomes. There are multiple strategies to manage ENI, with changing the EN formula being one.
Objective(s)/Process or Summary of Content
This practice survey aimed to understand ENI: prevalence, management of symptoms and EN formula features considered when changing formulas to manage ENI.
Method(s)/Systemic Approach Used
Canadian clinical dietitians working across care settings (n=4827) were invited to complete a 28-question, validated, online survey if involved in the management of adult and/or pediatric patients receiving EN.
Results/Conclusions
517 surveys were analyzed, representing adult and pediatric dietitians nationally and by care setting. Significantly more adult dietitians, versus pediatric, (83.4%, 59.1% respectively, p=0.0012) reported ENI occurred in <40% of patients.
Conclusions(s)/Recommendations
To manage ENI symptoms, assessing medications, elevating the head of the bed and changing EN infusion rate, volume or feeding regime were the highest ranked strategies. Most (>90%) respondents change the EN formula <50% of the time to manage ENI.
Significance to Dietetics
This study contributes to the understanding of current management of ENI symptoms in pediatric and adult clinical dietetic practice in Canada.
